Dependency Status Form

Use the checklist below to assist you in deciding your dependency status. Answering "Yes" to one of the questions below may qualify you as an independent student.

1) Are you at least 24 years of age?

2) Are you married?

3) Do you have any legal dependents that you provide at least 50% support for?

4) Are you enrolled in a graduate or professional program?

5) Were you an orphan or ward of the court until age 18?

6) Are you a veteran of the U.S. Armed Services?

Dependent: Student and Parent financial information is required.

Independent: Student and/or spouse financial information is required.
